COVID-19 Family Information

Nazareth Catholic Community is taking a number of steps to ensure that our community remain safe and well informed regarding the Novel Coronavirus (COVID-19).

As updated and important information becomes available for families, it will be provided via email and will be posted on this page as a reference. Please ensure your details are up to date.

A message to our community from Mr Baker:

 

My Child Has Tested Positive to COVID

If your child has tested positive to COVID please complete this form as soon as possible.

Please complete this form for each child who has tested positive should you have multiple cases in your household. 

We will then use this information to notify relevant staff and families. 

Please note that SA Health requires Nazareth to report COVID case information on a weekly basis. Data collected via this form will be shared with SA Health under these requirements.

If your child is unwell or absent for any other reason please call our dedicated absentee lines and your child's absence will be marked accordingly.

Access to Supervised Care - Term 1 Holidays

To access supervised care on Thursday 14 April and/or Monday 2 May, please complete the following form

Please note that supervised care will take place on the FINDON campus only. There will be no charge to families for accessing this care arrangement between 8:30am - 3:30pm. Before and/or After School Care will be available on these days. Please book in as per normal via 'My Family Lounge' and fees will apply. 

Students are asked to bring their lunch, recess, hat and water bottles in preparation for the day. We ask that students wear suitable sunsmart clothing. 

Please note that regardless of your current OSHC bookings, children requiring supervised care must be registered via the form above.
